This Privacy Policy explains how Breen Center for Optimal Health (“the Practice,” “we,” “us”) collects, uses, and discloses information through this website.

Two kinds of information, two sets of rules

Information you give us through this website — for example a consultation request form — is governed by this Privacy Policy.

Information created when you become a patient — your medical record, laboratory results, prescriptions, and communications with your care team — is protected health information (PHI) governed by HIPAA and by our Notice of Privacy Practices, which describes your rights in detail. What we collect

• Information you submit: name, telephone number, email address, and any information you choose to include in a free-text field on a contact or consultation form.
• Information collected automatically: IP address, browser and device type, pages viewed, and referring URL, gathered through cookies and similar technologies.

Please do not send detailed medical information through a website form or by unencrypted email. Website forms are not a secure channel for PHI. Use the patient portal or call the office.

How we use information

• To respond to your inquiry and schedule an appointment.
• To operate, secure, and improve this website.
• To comply with legal obligations.

We do not sell your personal information.

How we share information

We share information with service providers who host our website, manage our practice-management and electronic health record systems, or process payments, in each case only as needed to perform those services. Vendors that handle PHI do so under a Business Associate Agreement. We also disclose information where required by law.

Your California privacy rights

Under the California Consumer Privacy Act, as amended, California residents may request access to, correction of, or deletion of personal information we hold, and may direct us not to sell or share it. Medical information governed by HIPAA and the California Confidentiality of Medical Information Act is subject to those statutes rather than the CCPA. To make a request, contact us at the address below. We will not discriminate against you for exercising these rights.

Security and retention

We maintain administrative, physical, and technical safeguards designed to protect the information we hold. No method of transmission or storage is perfectly secure. We retain information for as long as needed for the purposes described here and as required by law.
