Telehealth Informed Consent

This document explains how telehealth works at our practice and what you are agreeing to when you receive care remotely. You will be asked to acknowledge it before your first telehealth visit.

[COUNSEL MUST CONFIRM this consent satisfies California Business & Professions Code § 2290.5 and the corresponding informed-consent statute of every other state in which the practice treats patients. Consent requirements differ by state, and several require documented verbal consent in addition to written consent.]

How telehealth works here

Your care begins with an in-person visit at our Irvine office — the Executive Physical — which establishes the physician–patient relationship and provides the examination and laboratory foundation for any treatment plan. After that, follow-up visits may be conducted by live video or telephone where clinically appropriate and legally permitted.

Depending on your history and treatment, we may require periodic in-person visits to continue care or to renew a prescription.

Where we can treat you

Our providers may deliver care only in jurisdictions where they hold an active licence. The governing law is generally that of the state where you are physically located at the time of the visit, not where you live or where we are located. You must tell us your location at the start of each telehealth visit. If you are outside a state where your provider is licensed, we may need to reschedule.

Benefits and limitations

Telehealth can improve access and reduce travel. It also has real limits: your provider cannot perform a physical examination, the technology can fail, and in rare cases a condition may go undetected that an in-person examination would have found. If your provider judges that telehealth is not appropriate for your concern, you will be asked to come in or to seek in-person care elsewhere.

Privacy and records

Telehealth visits are conducted over technology intended to comply with HIPAA. Your telehealth records are part of your medical record and are protected by the same laws and by our Notice of Privacy Practices. We do not record visits without your separate written permission. Please join from a private location.

Prescriptions

Prescriptions are issued only where clinically indicated after evaluation. Please read our Prescribing Policy, which forms part of this consent.

Your choices

Telehealth is voluntary. You may withdraw this consent, or decline a telehealth visit and request an in-person appointment, at any time without affecting your right to future care. You may ask questions about telehealth at any point.

Emergencies

Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
